Rehabilitative Therapy at Home
Our team of board-certified Physical Therapists strive to ensure that the performance of daily activities is made easier for our patients. Our physical therapist helps take care of patients in all phases of healing, from initial diagnosis through the restorative and preventive stages of recovery.
Our treatment plans are patient-specific meaning that our therapist evaluates and develop activities that will impact the individual patient.
Our end goal is to help our patients reduce the need for long term use of prescription medications by helping them maintain flexibility and mobility thereby increasing strength and endurance.
Our patients are also taught how to manage their health conditions and ways to mitigate re-occurrence.
What to expect:
- Undergo a physical exam and evaluation, including health history and certain testing procedures, evaluation of posture, movement and flexibility, and of muscle and joint motion and performance
- Receive a clinical diagnosis, prognosis, plan of care, and short and long-term goals
- Receive physical therapy treatment and intervention based on the therapist’s evaluation and diagnosis
- Receive self-management recommendations
